One of the delights of our office is that you can call and speak to a real person about when you would like to come in. We can personally fit your appointment to an ideal time and date.
Appointments - Online Requests
You can make routine, non-urgent appointments online with 24-hour advance notice using the form below.
Please complete and submit the form below with your requested time and date for an appointment, and one of our team members will call you to confirm.
Preparation for your appointment
When you come for your appointment, please bring your insurance cards and, if applicable, your eyeglasses for both distance and near vision, your contact lenses (and their boxes labeled right and left), your mobile phone, a list of your current medications and a bag of your prescription eyedrops.
Cancellation Policy
You may cancel and reschedule your appointment without a cancellation charge if you do so with at least 24 hours notice. Late cancellations and no-shows will be charged a $35 cancellation fee.
